  A sworn worksheet must be filed in every case involving children. Use the Ohio Child Support Calculator to compute the guideline amount.

Civil filings in this court are not made on statewide forms. The Clerk of Courts accepts the filing and can tell you what the office requires.

• Civil Complaint

  Provided by the Clerk of Courts — ask at the office or call ahead.

• Motion for Continuance

  There is no standard form. A continuance must be requested in writing and must explain the reason — it cannot be granted over the phone. If you have an attorney, they file it for you.

• Subpoena / Subpoena Duces Tecum

  Provided by the Clerk of Courts — ask at the office or call ahead.

Criminal forms are handled through the Clerk of Courts and at your court appearances.

• Plea of Not Guilty

  Provided by the Clerk of Courts — ask at the office or call ahead.

• Financial Disclosure / Affidavit of Indigency

  If you cannot afford an attorney, you will complete a financial disclosure statement to establish eligibility for court-appointed counsel. The form is provided to you by the court at your first appearance.

• Application for Court-Appointed Counsel

  Ask the court or the Clerk at your first appearance. Eligibility follows the Ohio Public Defender guidelines.

Transcripts are prepared by the court reporter.

Jury paperwork is sent to you with your summons.

• Jury Qualification Questionnaire

  Included in the packet mailed with your jury summons. If you need a replacement, contact the court.

• Excuse or Postponement Request

  There is no separate form. Send a written request explaining your reason before your service date — see the juror page for how to submit it.
